High Holy Day Registration Form- Members 2024

This Registration Form is for members who plan to attend services in-person. Below you will first be asked about services the adults will attend and then you will be asked about the services your children will attend.
Please Register by September 13th so that we can get you your tickets. 
Please enter the name of each adult. Then check which services that person will attend.
Please enter the name/s and check the box for services your 18-40 year old children will be attending.
They must have a ticket for services, so we must have all names. 

In the boxes below, please let us know how many additional adults, 18-40 will be attending each service. 
In the boxes below, please let us know how many children, from 5th grade, thru the age of 17 will be attending for each service.
